Simply follow these steps to easily pay by fax:

1. Complete the Gift Certificates Order by Fax Form below. Make sure to verify the information before clicking OK.
2. Print the credit card authorization form. Enter your information. Please be sure to enter the total charge amount, including the cost of overnight shipping if applicable. Sign the form and fax it to 954-523-1199.
3. Once we recieve your fax and process the payment we will contact you by phone or email, whichever you choose. Then we will deliver the completed gift certificate according to your instructions. If you choose to pick it up in person then it will be at the front desk.
